Four ways a cat gets round
Fat is the common indoor story. It pads the belly and lower back. You can often feel a waist with firm hands. The swelling is slow.
Fluid is the story that scares clinicians. Early on, many cats still eat. The belly may feel taut, like a water balloon. A “fluid wave” is a clinic trick, not a home game.
Stool and gas sit lower and can make an older cat look pregnant after two days without a dump. Pregnancy and pyometra add mass in the same neighborhood. Organ enlargement feels more localized. Clover’s pear was fluid plus a little fat, which is why appetite fooled Maren.
Heart failure in cats often shows as fluid around the lungs, but belly fluid happens too. Liver disease, some cancers, and FIP are on the fluid list. Parasites do not usually create a tight, sloshing abdomen.
What you can check without poking hard
Look from above. A fat cat is broad. A fluid cat can look like a drum and still have skinny legs and a skinny face. Sharp spine plus a swollen belly argues against “just chubby.” Count resting breaths. Open-mouth breathing is a reason to go now.
Note the box. No stool for 48 hours with straining is constipation or worse. Extra thirst points toward other disease. Ask the pregnancy question out loud for intact females, even indoor ones.

What the clinic will want
Bring weights and two photos. The vet will feel the abdomen, listen to heart and lungs, and likely run bloodwork. X-rays show fat versus a mass versus a packed colon. Ultrasound is better for fluid. If fluid is present, a sample splits the cause list in half.
Treatment follows the cause. Heart disease is not a calorie cut. Constipation may need fluids or more if the colon is stretched. FIP has antiviral options in many regions now. Fat needs a slow diet so the liver is not blindsided.
Do not crash-diet a round cat. Do not tap fluid at home. Do not wait for appetite to fail; appetite is a late messenger in some of these diseases.
Clover had a murmur Maren had never been told about and a modest amount of abdominal fluid. She still ate chicken. Eating was not the all-clear. It was the reason Maren had time to get there.
